AUTHORIZED MEDELA® DEALER FOR ALL RENTALS & SALES OF MEDELA® BREAST PUMPS AND MEDELA® ACCESSORIES....

This contract is between BABY MANIA Rental Station and the Lessee identified under RENTAL CONTRACT....

1. The Medela® Lactina® Breast pump equipment remains the property of Medela, Inc.,1101 Corporate Drive, McHenry, Illinois. Lessee has no rights to such breast pump equipment except as expressed in this Agreement.

2. Lessee may purchase a Lactina® breastfeeding accessory kit or Mother's Manual. The Lactina® breastfeeding Kit becomes the property of Lessee.

3. Lessee agrees to pay the Lactina® breast pump rental fees in accordance with the fee structure established on this agreement. The Lactina® breast pump rental fee is due in advance.

4. The Lactina® breast pump rental rates for the Lactina® breast pump equipment and the purchase price for the Lactina® breast pump kit do not include any applicable sales or use tax.

5. Lessee agrees to inform BABY MANIA Breast Pump Rental Station of any change of address.

6. Lessee agrees not to move the Lactina® breast pump equipment out of this State without the consent of the BABY MANIA Breast Pump Rental Station.

7. Lessee agrees to allow no person other than the Lessee to use the Lactina® breast pump equipment provided.

8. Lessee agrees to return the Lactina® breast pump equipment in clean condition. If the Lactina® breast pump equipment is not clean, Lessee agrees to pay a minimum cleaning fee of $5.00.

9. Lessee agrees to return the Lactina® breast pump equipment in good repair. If the Lactina® breast pump equipment is not in good repair, Lessee agrees to pay a minimum repair fee of $30.00.

10. The Lessee shall be responsible for all reasonable legal fees and other costs involved in collection of overdue amounts and/or recovery of leased breast pump equipment.

11. The BABY MANIA Breast Pump Rental Station has the right to cancel this agreement at any time with THREE DAYS NOTICE....
